Go See, Think, Do (GSTD) Problem Solving Activity

Has anyone here ever used a GSTD (Go See, Think, Do,) Problem solving activity? I am looking for a GSTD form that can be used for a team that needs it to help them with getting to RCCAs.

Is a customer requiring this method? Are you just looking for a simple problem solving method suitable for use on the manufacturing floor? If you are open to using other methods, I recommend the Toyota 8-step problem solving process and display it in a Toyota A3 format.

I showed the customer how to break problems down into 3 parts (Go SEE, Think & Do) They actually loved it a lot because they feel intimidated a little with the 8-D tool which is what their customer requires them to use. The GSTD form would be good for them to capture all of the activities they did to solve the problem and all of the actions that may spawn from the activity .

I think that I might be able to modify the A-3 into a format exactly like the GSTD form.

Don't confuse the Toyota 8-step approach with the Ford 8D approach. They are distinctly different. The Toyota approach focuses on breaking big problems down into smaller, more manageable problems.

This is what I came up with from the A-3 form. I am not completely finished making adjustments to it. Having trouble getting the page breaks to be where I want them, but the supplier loves the concept and plan to implement it.
