What is Prevention in FMEA?

On page 49 of FMEA 4th
Prevention :
Eliminate (prevent) the cause of the mechanism of failure or the failure mode from occurring, or reduce its rate of occurrence.
Does this sentence means:
1 Eliminate (prevent) the cause of the mechanism of failure or eliminate (prevent) the cause of the failure mode from occurring
2 Eliminate (prevent) the cause of the mechanism of failure or eliminate (prevent) the failure mode from occurring?

Which one is correct?

Hi everybody,
if prepared work instruction and displayed on station,Can it be my prevention control in FMEA?

I would think of accessible work instructions (and training) as a control to reduce potential risk (i.e. to reduce (Occurrence x Detection)), but I would not think of it as prevention of that risk. In this sense, I think of 'prevention' as reducing (Occurrence x Detection) of a specific risk to zero; i.e., making it impossible to make that mistake or not detect it, due to design/tools/jigs, etc.

OK, so if you have an implant, you can never "prevent" or "eliminate" the risk associated with an infectious agent hitching a ride into the body on your implant? However, you may be able to "prevent" and/or "eliminate" the cause of a specific type of sterilization-related failure?
