Supplier/Vendor Surveys (360)

We are currently looking at implementing a Supplier/Vendor 360 survey process. Instead of Voice of the Customer looking at Voice of the Vendor. Looking to see if anyone has any experience with this and if they would share some of the questions their survey is asking? Just curious to see what other out there are doing and looking at.
