I am training and have been asked to prepare the questions I would ask the CEO of a company whilst conducting an interview as Lead Auditor on the 2nd certification stage of a 3rd party audit can anyone help please?

Look at this post + attachment. It is an ISO 9001-based document, but possibly applicable to what you want to do.
Use it to generate some ideas for the audit.

Doesn't make sense to me because I just did that in real life and you know what....I never know what I'm going to ask until I have something to ask a question about.

And here's another fact of life....You can at best only expect to be able to ask your 1st question...Why? Because your 2nd question is purely dependant upon the answer to your 1st and what you get may or may not be what you expected or planned for and the result can be your tossing away your planned questions because you have to follow a completely different path.

All audits are unique and all interviews are unique and if you don't have the basic stuff in your head to begin with maybe auditing isn't your thing.

You are speaking of BS OHSAS 18001:2007, (Occupactional health and safety management), right?
I hear there is a UK and an international flavor.
Your question(s) are shooting at the top of the company, so reasonably would be the same for almost all standards.
1. Are you serious about this implementation?
2. How serious?
3. Speak to me about your management review, processes, & plans.

What kind of answer would you expect to questions 1 & 2?
How do you quantify "serious"?

From an auditor's perspective such questions don't give very revealing answers, George!

Since each auditor is an individual, their line of inquiry is also likely to be very individual. Although there maybe a flavor for the style of question, trying to 'prep' top management to have answers to questions they may/not be asked is impractical and they will be overwhelmed by the (real) variety.

What's better is to prepare them to give a short dissertation on how the implementation of the system has helped reduce the number of reportable accidents, raised awareness of risks and so on...

I guess the word, and question serious? could be viewed like "Kill me now!"
There is the 5Ws why, why, why, why, why of root cause analysis.
I suggest the 5Hs of serious: How, How, How, How, How.
The 5H might be alot for some CEOs, and this would answer the how serious question.
In agreement with an earlier post, It is a question of a question of a question audit method.
I hope this might be more in line with a course of questioning.
It might be my only , nope! I checked, I have another for later.
I might ask, is everyone safe in your company? (how do you practice safety?, Is your safety only LEAN-safe*)
*LEAN-safe, would be part of LSS, and just enough. Not a great type of safe.
How safe? (safe enough? safe to standard? zero tolorance? just a few died?)
How do you know? (who told you? how often?)
How do they know? (what is their reasoning?)
How does the report prove this? (is this number biased or weighted to look good? how much? where?)
How is it validated? (is safety spoken, a faith, or a practice?)
I would like to hear the word safety/safely in the same sentence which describes a job each time.
It would be correct to ask if the employees all got a Merry safe Christmas wish from the CEO.

In the end the CEO is in charge, and just asking him what his course and strategy are is my best answer.
As in my earlier post. He is at the top, and all questions for him are answered in course and strategy.
