As to whether you have to call the APQP team together - well, if still in the design stage this is correct - typically the design team / APQP team would address the issue. However, I really think this is meant more at changes requested by the customer during production. In that case, you would call together an 'appropriate' team. Your system should define how you handle incoming customer requests. In a lot of companies these come in through sales and sales coordinates. However you do it, make sure you have a system defined.
