The 5-why strategy is not as simple and easy as some proponents and teachers have presented it. Sakichi Toyoda intended it to be executed at the actual place(s) of the problem, accompanied by evidence and to enable the investigator to go beyond surface explanations of the cause of problems. I believe the line is "When you believe in things you don't understand, then you suffer" - and it was written (and first performed by) Stevie Wonder. Now, "why" was that mistake made? :)

Regardless, good info - especially the part about needing deep knowledge. I've also seen where the last couple of "whys" are forced just to get to 5.
 
Part 2 should tomorrow. A few minor corrections to part 1 as well - not effecting the message.
