The results of FMEA go to whoever has the power to remove the root causes of failure.

Failure Mode Effects Analysis (FMEA) - ASQ

So, your list should exclude 3 and 6.

...and include process and product design managers which I’m guessing is 4.

I’m not sure what you mean by “responses”.

Do you mean:

• Customers that could be impacted by a potential failure mode? (to determine severity). Or,
• Customer (beneficiaries) of the FMEA Process?
