Best practice is whatever works best in your organisation. I haven't voted and won't because I could answer Yes to each box for a different client. It depends on how big your company is, and what will work best.
I do agree with Howste:
Me neither. And I doubt that s/he either wants to or needs to approve all 2000 of them. (That's a heck of a lot of procedures!!)
It usually makes sense for the CEO/MD/Board/whoever to approve the very top-level documents, eg, Policy. But all the procedures? Only in a very small organisation and where they really want to remain very hands-on. Delegation to an appropriate level of SME and authority is wise.
Should these procedures be approved by HOD or HOD as well as CEO? Previously CEO was specified in 4.2.3, but later removed because of the burden.
