Can I hear some folks weigh in on ISO9001 requiring a Preventive Maintenance Program (whether documented or not...)? I fail to see where it is called out as required (like 13485). I am not looking for whether its "good" to have one but rather if it is required.

The ISO 9001 does not require the organization to have a preventive maintenance program. ISO standards do not prescribe any technique that has to be adopted, and so the ISO 13485 smartly uses the work "interval of performing the maintenance activities"
Where necessary, the NOTE in the end of the clause description will indicate towards the techniques that could be adopted by the organization.
It gives you the freedom and triggers the risk based approach.
Now it all boils down to your business, your industry and the risks that you can foresee to apply preventive maintenance program where necessary.
