I am conducting an internal audit of our AS9100D QMS. Since you must have a record of training, must the training form be a released document in your QMS or can it reside outside the system? This is for a special process, X-Ray.

The completed record is being retained as evidence of training, it has a form number, revision and date on it but it is not an OJT form in the QMS.

You seem to have described some of the controls for documents that are subject revision.

Records are different types of documents which in this case would be controlled by whatever is necessary to make them retrievable (location, type and employee name - for example).

So, I'd expect to see the record showing the name of the employee, a description or cross reference to their specific abilities, skills and knowledge, when and by whom the competence of this employee was verified. Naturally you may also look at the verifier's evidence of competence.

Your procedure may specify the form to be used or it may allow a free format and that's fine provided you can see evidence of competence.
