AS9102 Form #3 modification

I have not seen this in any of the forums.
Can I modify the AS9102 Form #3. I want to compress column #8 "Requirements", so that I can expand Column #9 "Results". I want to add 3 column for #9 (this is a concern with my CB) so I can record/enter a "Range". i.e. a 6 piece sampling of a .062 hole dia is .060-.062-.064. As opposed to recording 6 different entries.

Per AS9102B, 4.7.1c:

Forms other than those depicted in Appendix B may be used; however, they shall contain all "Required" and "Conditionally Required" information and have the same field reference numbers.

This means that yes you can alter the forms if you need to so long as you maintain the references and field numbers. My FAIR has several columns that are part of the requirements and results section.

I am not exactly sure what you are asking about when you say "internal control number". If you are asking about Form 1, item 4 (FAI Report number) then I use the part number followed by the date the FAIR was finalized.

If you are asking if our FAI form (excel file) is a controlled document, then yes it is and I even wrote up work instructions on how to fill it out.

Per AS9102b Paragereph 4.7.1

b. Forms contained in Appendix B should be used to document the results of the FAI.

c. Forms other than those depicted in Appendix B may be used; however, they shall contain all ?Required? and

?Conditionally Required? information and have the same field reference numbers.

d. Use of forms from a previous 9102 standard revision (e.g., AS9102A) is acceptable, provided the current form instructions are utilized.

Yes, you could additional columns or split columns to provide additional information required by customers and / or internally. All you do is keep the number and add a letter to it so that the original intent is not eliminated or changed.

Yes, as long as the forms that are used contain all the ?Required? and ?Conditionally Required? fields with the same field reference numbers are on the forms, then they may be used.
