Attribute Gage R&R Form - Long Version - AIAG's MSA Manual

Does anyone has a template for the long version Attribute Gage R&R.
Has anybody good reference literature on the subject (I do have AIAG's MSA manual, but like to read others).

I've get your attachment on attribute MSA.XLS,but the file's attribute is set as "protected",so i cann't know how did you calculate those values out, like how to calculate the UCL and LCL.
