According to a latest poll within 270 construction professionals on Linkedin, it seems that bad communication within the teams on site is the main generator of NCRs in a construction project...

The result doesn't surprise me at all...

Maybe that's why the house I am trying to buy has a window over the master bedroom's bathroom sink instead of the bath tub, which is the typical placement.

Its a pity that lack of compliance to defined process requirements was/is not a selectable option in each of these polls.

I think most covers would agree that quality theory says we need to focus on the process not the person, the 80-20 rule.

Its only in safety that the 80-20 rule is applied the other way round, hence behaviour based safety and to a lessor extent; human factors stuff which supports BBS.

Though project-oriented work is one-off, root causes in this type of work are similar to those in more repetitive processes: they reveal specific corrective action required to eliminate recurrence.

Generic causes, like those in the poll, aren't very helpful in this regard, so I'd hesitate to call them "root" causes at all. Perhaps "root cause categories"? Even so, thinking of root causes this way seems misguided to me.

I did not like the choice of "root causes" either.

For all we know the contractors were not selected for their ability to manage quality. Or they were not required to continue their effective use of management systems conforming to their national standard for quality management systems.

Who knows?
