Certificate of Conformance Form - COC for each customer a controlled document?

New the forum and have a question about COCs. I have a standard COC that is a controlled document REV controlled and I have multiple customer that require different items on the COC. The question is do I have to make each COC for each customer a controlled document with REV control or can I have the one main format REV controlled and add needed things per customer?

The latter; i.e., the form is controlled, not the certificate itself. CoC’s are, with very few exceptions, non-modifiable documents.

We have identified a few of our forms that can be edited freely by certain personnel. The main two being our CofC and our Tool Inspection Report. The Quality Manger and Lead Inspectors are allowed to add whatever they need to satisfy customer specific requirements, but nothing on the original form may be deleted. In these instances, those people have been trained and given the encryption password.
