Matrix is to be prepared by each company individually.
First, system processes need to be documented and activities within processes can be matched with standard clauses.
Matrix can't be department based, it must be process based.
From 2015 process approach is a law.

As specified by @Sebastian , such a matrix differs from organization to organization, the reason being the organization determines which activities will be done by which processes (called as department earlier).
