A very similar event took place exactly here where I live, few miles from my home. A junk yard named Yonke Fenix allegedly received for recylcing a decomissioned X-Ray or a cancer treatment equipment from a local hospital
The unit was melted along with other scrap products, and several products were made with the iron which was contaminated with Cobalt 60
Then the products were sold into the USA and Mexico, until the contamination was discovered.

No when I teach ISO 14971 (Risk Management) I explain what life cycle means using this example and how the risk associated with Medical Devices does not finish until the dvice has been properly decommisioned
