Reference - usually a clause from the standard and/or the para of the procedure
Statement of fact- state clearly what the problem is. Phrases such as 'evidence of ....' or 'no evidence of .....' are usually good to use. Make sure to stick to the facts and don't make (or imply) generalisations unless you have evidence that what you have seen really applies to everything.
Objective evidence - list a couple of examples of the evidence you witnessed.
Scale of the problem - not essential but can add a degree of clarification e.g. 10 checked, 7 without references stated.
But what I was looking for was help on filling in the Correction, Root Cause, Corrective Action side. Reading through, are there usually redundant statements, or frequently used ones, etc.?
