If you go to https://www.cjsys.net - Link was: /demo.htm - you can download a demo of an Access database that is used for Document Control. I have used this application in many companies and have never had a problem with document control.

Controlled Document Distribution Log

We are confronting problems with our company's controlled document distribution log.

We are trying to improve our form but it seems that it is to cluttered.

Can anyone help me with a sample of a form that covers almost all needs without having to use more than one log?

I will appreciate all the help I can get for this matter.
