Forms must be controlled only when prescriptive in nature.
More specifically, one would need to control a form which provides space to record inspection results while detailing acceptance criteria for the characteristics being assessed or one that specifies which characteristics must be assessed. This need for control exists as a failure to implement changes to the form's content may result in required information not being recorded or in the acceptance of nonconforming product.
Where the characteristic to be assessed or its acceptance criteria are defined within a work instruction or other similar document the form need not be controlled as its format is immaterial.
