we follow a 24/3/10 process that includes within Step 7 a preventative step. take a look and let me know what you think.

Is it too complicated for general use?
Should the preventive action be included in a separate form (probably not)?
Reference process control plan and revise?
Note any reviews and updating of FMEA documents?

I guess I am saying to follow back through the entire process to the beginning and the documentation needed properly assess the process. It sounds difficult on the surface but can be initiated by 1 person and acted on from there.
