I am new to this forum. However, in my company I use one form for non-conformances. The NCR form has a starts by stating what the non-conformance is and identifying the root cause. Immediate corrective action must occur withing the first 478 hours. Then a date is set for long term corrective action of that problem.
I have a separte form for Corrective & Preventive Action. This would be for chronic or more major issues and problems.
