Sorry, actually I am giving compliment on the 8D report which is very detail and useful. What you have here is what I need. Thank you very much.

If you use an 8D format for your corrective actions do you also need a preventive action form? If not do you need to identify the 8D as a corrective/Preventive form

You can use the same form for corrective or preventive actions. Just put a checkbox on the top to indicate whether it is a corrective or preventive action. The fields are the same. The only difference is the corrective action will be for issues that have occurred. The preventive action will be for issues that may occur.

Typically when an 8D is completed it is in reponse or reacting to a specific issue. With that said a preventive action form is not required, and the 8D can be used as preventative action for other similiar parts or processes.

I'd be interested to see these forms as well.

We tried a simple - "Combo" type form - Used a box checkoff to identify what the form was addressing Cor/Prevent/ Non-Com only but it did not work well for us.

Frankly IMHO would suggest you keep them separate.

We found it to easy to confuse the issues when reported on one from format. What happened was both Corrective and Preventative went un-reported because of the confusion....not only when filling out the form and deciding what was Corrective - what was Preventative - but in explaining how we handled and recorded the info to our external auditor.
