I don't know if George is still here or not, but if anyone is interested, I have an MS Access database I ponyed up a couple of years ago for our quality system. I based it off the 7-Step Corrective/Preventive Action form we'd been using for some time. We use it for both corrective and preventive action, and base it on the 7 step system: Problem Description, Interim Action/Containment, Root Cause, Permanent Action, Verification, Control, and Prevention.
The database has an entry form "store front" with three tabs giving three different data sections. It has some buttons for new action, closing, a full report, and a report for a graph. Also some email notification buttons for specific situations. It automatically assigns an ID number to each entry (starting with 1, and going incrementally up).
It's about 1/2 MB (hopefully does not get much bigger with CAR's), so it shouldn't be a big deal to send, if anyone is interested. It's kind of geared more towards our processes, but if any of you have an Access-knowledgeable person around, it won't be very hard to modify to your own situation. If I knew how, I'd attach it here for you to download from this website (maybe someone can help out?).
