Customer discovered electrical failure

Electrical failure passed

I have a doubt, I have a process of wire assy that my customer detected with a electrical failure. The problem was a mis wire. This cable had the electrical stamp for approval. Due I have a low volume how this could occurred.
Could you give me some ideas? The electrical test is working well and the stamp is placing by the operator.

Whenever you have an operator test something and put a stamp on it, you will have the chance that the operator may make a mistake. This is because the operator can get tired, or bored or can forget to test. It is better to let the machine doing the test apply a mark to the wires. It is fairly simple today to rig up a solenoid which can apply a mark when the test has passed.
