Detection Criteria Examples - PFMEA (Process FMEA) - Emergency Safety Fixtures

Again our company is new to the FMEA process and need some help to understand and differentiate between some of the lower number PFMEA detection rankings.

From 10 to 6 the rankings are clear to us but from 5-2 ,some have questions. Wondering if someone could share some examples of process controls which fall into each of these detection ranking numbers.

Background:
Our company is an assembler of emergency safety fixtures so our needs for detection & occurence need to be low to offset our high severity rankings if the products do not function properly.

This board has been an amazing help to us already.
Thanks

Re: PFMEA - Detection Criteria Examples

Remember that each failure mode should be evaluated separately. The evaluation criteria should be specific to your process and the failure modes identified. You shouldn't blindly accept someone else's rankings without looking closely at your product and process. That said, here is a "guideline" offered by the automotive industry.

5 - controls may detect - Control is based on variable gaging after parts have left the station, or Go/No-Go gaging is performed on 100% of the parts after leaving the station.

4 - Controls have a good chance of detection - Error detection in subsequent operations OR gaging performed on set-up and first-piece check.

3 - Controls have a good chance of detection - Error detection in-station, or error detection in subsequent operations by multiple layers of acceptance where a discrepant part cannot be accepted.

2 - Controls are almost certain to detect - Error detection in-station (automatic gaging with automatic stop feature) Cannot pass a discrepant part.

Hope this helps.

Thanks for your thoughts Dave,

After thinking about the failure modes 2-4 from AIAG a little more it became easier to understand their meaning. We were able to move through the detection section of our FMEA efficiently.
