please help me about means of patient protection and test voltage
related to defibrillator device.
main capacitor charge to 1500vDC at 200j.
how much insulation should exist between paddles of defibrillator
and main capacitor? ?mopp ?vrms or DC

Have you read IEC 60601-2-4? That standard has a bunch of special requirements for different parts of the defib circuit.

It's not simple any you should be laying out in a block diagram. There are different circuits including the paddles, capacitor bank, ECG monitoring circuits, control circuit and so on.
