do you need to update PPAP due to a discrepancy

Do I have to update the PPAP after every kind of discrepancy. For example: We may detect a discrepancy during runing production when following the control plan. After that we take actions to correct it. We scrap the parts (there were not many). Do we have to update the PPAP.

The PPAP has to be updated to reflect the changes made in the product and/or process.
