The trigger for this was an in process inspection. We found non conforming parts which triggered a CAR. The Root Cause was traced to a line person who had modified the procedure to make things easier for herself. She did not go thru the procedure modification request process. This actually triggered another CAR for training. The reason I am concerned is that our certification audit is coming up shortly and we will have a CAR with no acknowledgement of a problem in our Internal Audits. The problem didn't exist at the time of the IA.

You cannot hold audits responsible for finding everything.
It's a
system , remember? And a different part of your system (in process inspection) found a problem and triggered a CA. Cause analysis triggered another. Sounds like a system that's functioning just fine to me.
