I doubt an external auditor would question your internal audit effectiveness for that. As you said yourself, the problem did not exist at the time of the internal audit. From what you describe, it seems to me that you have a good process in place. Just trigger the CAR(s) and let the process follow it's course.

The Root Cause was traced to a line person who had modified the procedure to make things easier for herself. She did not go thru the procedure modification request process. This actually triggered another CAR for training. The reason I am concerned is that our certification audit is coming up shortly and we will have a CAR with no acknowledgement of a problem in our Internal Audits. The problem didn't exist at the time of the IA.

Hey, there was a problem and you found it!
That's good.
You cannot hold audits responsible for finding everything.
It's a system , remember? And a different part of your system (in process inspection) found a problem and triggered a CA. Cause analysis triggered another. Sounds like a system that's functioning just fine to me.
