Hi to all!

I have my existing Document Change Form, we are in a small company and I want a simple forms in asmuchas I could. The form that I have is a sort of request or approval for a certain change in the document or in a system.

Now, I want to make a Change Notice, it is a sequel to the form above wherein this form is Notice that the change is approved and is for implementation and the information dissemination.

Do you have any similar form that i could get an idea of. Thanks alot.:rolleyes:

Why bother with another form? Just send an e-mail. You could also simply add an approval section to the change request form and then send out a copy of the approved form.

WHAT document is changed
WHERE in the document the change is made
WHY is the change now necessary
WHEN is the document change being effected from
WHO is approving the change release

What type of documents does this apply to? Process, design, all of the above....

The change notice should include implementation information. If process change, is training required, new equipment, and so on. If design, are new parts required, what is the production impact, what is the serial number cut-in.

All of these issues should be considered and documented in the change notice.

What is the purpose of Document Change Notice? Is it to notify the concerned groups or individuals of the revised document, or to notify of the document to be revised?

Thanks for the reply. I want to use it both. Notify the concerned group or individuals and notify of the document to be revised.

Am I not doing a redundant form, if I have already the existing Document Change Form which i attached in this thread?

I had not looked at your attachment when I first replied, but now it looks like you are creating a redundant form. Why not just send out a copy of the approved request? Change the title to If it is a partial approval, note that on the request form. Change the title to "Document Change Request / Notification" if you like.

You say you are a small company. How small? I question if you even need a form. I have adopted a similar one in the small shop I am in now. I find it quite a time waster to have to fill out a request form and send it to myself for my approval so I can make the change I requested, and then approve it myself. It will be going away once I get around to revising the document control procedure.

Don't invent bureaucracy because you think ISO requires it. Keep it simple.

Thanks a lot! We are just 11 personnel in the company, I am the one who's revising the document or any system in the production but I have the general manager who's approving my changes.

I might have been creating a redundant form! But I want to have a document that the people in the production has been informed about the change in the system and have their orientation attendance.

Sometimes it goes crazy when I always thought of having a ISO pattern in our small company, because I know someday that our management will plan to have a ISO certification, and most of my documents are patterned in such. As quote your word "
Beaurucracy ".

I like it! I'd add some more scenarios to it - like how to deviate from the existing document, while another one is 'tried out', for example. I also use a doc change form for initiating a new document or obsoleting an old one. It really is a great tool, if designed properly and used well. Of course, it's nothing new, product design changes serve a similar purpose. They are so much better than keeping change history in the actual document and what a mess emails would cause if you tried to track all that stuff!

I'd say a good start in the right direction, with a bit more work it would be very effective!
