Be aware, there are new ways to think about Doc "approvals." In most cases, if you make little changes to a document, there is no real need for a dozen people to approve the change. If it is a tiny change, is anyone going to veto it? And, if they do, you can change whatever they are objecting to.
If it is a significant change to the document, wouldn't the process owner have review the significant changes with other process managers, to make sure everyone is on board with the changes before he commences to writing the changes? So, even there, the changes would probably already be agreed to, before the document was changed.
A lot of companies have the Process Owner (Document Owner) and the Doc Control Coordinator do the official approvals, and other people in the loop are simply informed there was a change. This won't work for all companies, but probably would suffice for many smaller companies.
