Does a Document Distribution List need to be a Controlled Document?

Quit beating around the bush and use ISO 9001 for your answer:

If you have applied this...4.2.1-d) documents, including records, determined by the organization to be necessary to ensure the effective planning, operation and control of its processes.

Then the list must be controlled because it is a system document. There are no if's, and's, or but's

If it doesn't have to be used then why have it at all? If it has to be used or it serves a purpose, control it....And don't use the weak excuse of "Reference document"

I think there is a risk that by asking questions about details without sharing the bigger picture you may get misdirected.

What's your DDL for? Who else reads the controlled documents regarding patient care in addition to the visiting care workers? In my experience, a DDL is not kept with the documents it controls, but by the document control people who manage changes.

Why is controlling documents regarding patient care okay, but controlling DDLs "a hassle"? (And if it's a hassle for you, what's it like for care workers?)

I fear that ISO 9001's document control concepts may be too simple for your requirements and while they can be applied, it takes expertise. You might want to consider the more advanced concepts of "configuration management" which is an engineering technique for managing sets of related documents. I suspect your DDL may be more like a Bill of Materials, or in software a "make" file, than a distribution list: the people who need to be informed of a change to the care of patient x perhaps depends upon who has decided what needs to be changed with regard to the care of patient x.

Either way, previous posts are right: if any doc carries useful information it needs to be controlled.
