Do anyone in the group have an example of management of change format for EMS system?
example :- if there is a change in the process , how does this change effect our EMS system including aspect & impact , legal requirement etc.

I am not familiar with a particular format for managing change in EMS. My clients have used many tools, some of which were software but most involved documentation they already had in the form of MS Office documentation: Word, Powerpoint and Excel. I do expect to see consideration of regulatory and other requirements (now called "interested parties") as well as risk, environmental aspects and impacts, roles and responsibilities, processes and procedures, training and communication, infrastructure, measurement, indeed the impact of change should be considered for most of the system it impacts.
That sounds confusing because I do not know where you live or what kind of change you are referring to. I did find an article by EHS Today:
