The kind of form you use is decided AFTER you determine the process necessary for your organization.
In some organizations, inventory control is extremely tight and cost conscious, so every withdrawal of inventory or use of machine time and "expendables" by design, model-making, prototype, and testing functions of an organization must be tallied and charged to an appropriate account. So the form in those organizations is essentially an internal purchase order for materials and service, with appropriate places to indicate what account to charge.
In many organizations, especially smaller ones, designers and engineers work with scraps and free samples from suppliers and occasionally take excess raw materials or components or finished goods overstocks from inventory to use in models and design experiments. They are careful to reconcile inventory records so no one relies on an erroneous inventory in planning to manufacture or sell goods. In such cases, no requisition format is used, the designers just prowl and scrounge.
If your organization is very tightly controlled and scrounging for materials from inventory is not simply a stroll to the warehouse and scrap bins, just use your organization's purchase order form as a template. "Ship to" is the responsible engineer and "Bill to" is the project or account the cost is charged to. Te purchase order is handled by warehouse staff exactly as if it came from an outside customer; the only difference is the accounting.
