Too often, it seems, that we are trying to please our auditor.

...or second guess what they are looking for.

Instead we should be sure that we have an effective system and why.

Audits go well that way and after a strong defense we welcome well-crafted nonconformity statements that highlight system weaknesses overdue our corrective action.

A challenging question we might ask ourselves is "How do we measure the continual improvement of the effectiveness of our management system?"

First you need to define what is the management system, and how could I "see" that it is effective. Dr. Bill Bellows developed an interesting "Red Pen Blue Pen" exercise that helps you visualize an effective workplace (versus an ineffective one). Exercises like that can help you define "effectiveness". Then you need to see how you will detect the effectiveness, and what measures we could use the quantify (or at least categorize, such as the Baldrige scoring criteria). Then we can take periodic observations / measurements and see if it indeed continually improving.

Everyone has good comments. I understand the requirement to mean that you should be able to get your goals from your quality policy. That is how we did it. We always receive good remarks about the format from our auditors both for ISO and API. Including a generic copy of what we do.
Very good example taken from the Quality policy to the Objective.
we have to focus on Quality Policy what we wants right now for such a time and may be change the quality policy after some time.

I was recently told by a registrar auditor that 4.4.1 essentially requires that each of my individual key internal processes have at least one KPI specific to that process to monitor/measure its performance. He said that an overall company goal (i.e. OTD or shipped quality levels) is not adequate for processes like quoting or configuration management. For example, quoting might have a KPI of # days from customer request until quote is issued or # quotes converted to orders, etc.

Is this the general consensus?

You need to ask for the specific "shall." I see nothing in 4.4.1 that calls for KPIs of any kind.

4.4.1 doesn't say "KPIs", but it says performance indicators. Combined with 6.2, you have the guidance. Of course, you can take the verbiage in the clause and find the loophole if you wanted.
