We've used a very basic Excel file with columns for (1) Customer, (2) PN, (3) Rev., (4) Type (L1-4), (5) Reason, (6) Date Requested, (7) Target Date, (8) Date Sent, (9) Date Approved, (10) Date Rejected, (11) Comments.
