F-108 Request for Expansion of Scope of Accreditation

Hello, I recently took over as Quality Manager of a testing lab. One of our local documents references "F108 Request for Expansion of Scope of Accreditation" but I don't see that form on the A2LA site. However, on the Customer Portal there is a Scope Expansion Request button. Does this take the place of F108, then assuming F108 is not longer available to use?

In case anyone is interested, I was able to reach out to A2LA and was informed that F108 made obsolete on June 21, 2022. They are checking to see if there is something that would take the place of.

Thank you for the update. I know ANAB has this type of document, but I have not worked with a2La for many years, so am uncertain of their doucmentation.
