Failure Modes for Public Transport Vehicle Door Assembly - Your inputs please!

Hello, I am currently trying to set up an FMEA for our company.
In this plant they are assembling doorsystems for public transport vehicles.

I listed all process steps, from part receiving to shipping, and now I am trying to determine the potential failure modes.

For the receiving and assembly proces I currently have the following failure modes:
Receiving and incoming inspection:
Parts are damaged.
Mis-labeled.
Missing label.
Wrong quantity.
Bad part passed inspection.
Parts are not according to specification.
Handling damage.
Placed in wrong location.

Assembling:
Part not mounted.
Wrong part mounted.
Part is not in accordance to specifiactions.
Handling damage.
Incorrect fasteners used.
Part is fitted to loose / over torque.
Step is not performed.
Wrong glue used.
Defects are not noticed.

These are the points I could think of.
What is your view on this, is this good, do I miss something or is this totally wrong?

Thanks in advance.

PS. Sorry for my bad English.

Re: Failure Modes for Public Transport Vehicle Door Assembly

Thanks for your reaction.
I forgot to mention that I am making and Process FMEA and not an Design FMEA.
I already looked into that example before, but could not extract any information since it's an Design FMEA and about spraying parts.
In this plant parts are not getting adapted, it's just mounting parts for the doorpanel with screws, bolts and glue.

If you can describe the step by step flow of this assembly process, it may be easier for others to share their opinions.

Re: Failure Modes for Public Transport Vehicle Door Assembly

Pre-mounting
Study drawings
Take parts from order car
Mount "Flapframe side" to "Flap frame extrusion" [2x M6 torx socket button screw ]
Mount "Flap frame inside" to "Flap frame extrusion" [1x M6 torx socket button screw ]
Shove "Tapping strip" in "Flap frame extrusion" and secure [2x M8 torx socket button screw + locking disk spring]
Shove "Tapping strip" in "Horizontal doorprofile" and secure [2x M8 hex head cap screw + locking disk spring]
Put the parts back on the order car

Assemble frame
Put 2x "Vertical doorprofile" on worktable
Mount "Horizontal doorprofile" [4x M6 Socket cap screw]
Mount "Flap frame" [4x M6 Socket cap screw + locking disk spring]
Check for flat connection between different profiles, possibly adjust
Mount "Coverplate" [4x M6 Socket cap screw]
Run through test- and inspection report
Put doorframe on waiting frame

Glue glass
Put doorframe on worktable
Apply sealing tape around the doorframe
Apply primer on the doorframe [Prep L]
Clean ceramic screening on the glass [Cleaner I]
Check glass for deviations
Mount "Handle plastic" in the glas (1 Nm) [2x M5 hex flange nut + Loctite 243]
Apply kit on the doorframe [STR 360]
Position glass on the doorframe
Press glass to sealing tape and secure with tape
Run through test- and inspection report

Awwww....the good ol' days when I worked for a public bus manufacturer. I'm curious now if we worked for the same company or the competition. Feel free to PM me to resolve that issue. ;)

That being said, having developed some of the documentation for the manufacturing processes, I did much of it with a cross-fuctional team. I trusted THEM to be the experts and my role was simply to facilitate the process. Could you do the same?

I think it could be useful to see the entire sheet of your P-FMEA to have an overall overview.:bigwave:
The FMEA isn't written in English, so i think it won't help anything.

I don't think we've worked for the same company, since this one is located in Europe.

The failure modes listed in the OP are verified with them. We are not sure if we are doing it right like this.
