Failure Reporting

Imagine if you would that you had all the information relating to a product, i.e statistical analysis, concession and rejection reports etc.
How would you determine what to take into consideration when reprting on a failure, such as how to takle the problem. I have all the information from over the past two years about a part, spc analysis concession atc and my employer BAE Systemsa want into put in place a new failure reporting process that takes the information and suggests a course of action? your help in this matter, as always, will be very happily received. Or perhaps any books on the subject would be great

Unfortunately, I believe you'll have some difficulty if you're wanting to develop an 'automated' process. Typically there are too many possible combinations of factors which must (should) be taken into consideration.

-> statistical analysis,
-> concession and rejection reports etc.

And much more. Prints, work instructions - you name it - there's a lot you can call upon for an investigation.

This is the idea of a corrective action system. You take all the data and decide upon a course of action.
