FMEA Form - Clarification on correct way of filling up (key/fmea date, rev)

Hi

I would like to seek clarification on correct way of filling up the FMEA form
for these items :

Key Date: - I always input this as the PPAP submission date & will not
change this even at leter revision.

FMEA Date(org) : 1st Rev of FMEA
(rev): Latest Revision Copy
( this will be changed with each revision)

Are these correct??

There are slight differences between design and process FMEA.
