FMEA Severity 9 &10 - Example of "with warning?"

Hello to all! So much great advice here in this forum and I am still very new here and just a couple of years of work experience.

I am trying to explain FMEA to a few colleages and I can not describe the difference between 9 and 10 as described with warning and without warning respectively. These definitions are all over the Cove and the Web but I can not find an example of "with warning" that does not include a control measure. Control measures are later in the detection and i do not believe they should be included in the definition of severity. Am I wrong? Can someone give me a couple of examples of a 9 "with warning"?

Thanks for any help you can give!! Would really appreciate it!

Re: FMEA Severity 9&10 - example of with warning?

You can see some simple examples every time you start your car and all of those warning lights briefly turn on.

Re: FMEA Severity 9&10 - example of with warning?

Thank you very much for your response and example but I still don't understand.

Aren't those lights methods of detection for a failure mode and not a way to define severity?

Re: FMEA Severity 9&10 - example of with warning?

Think about oil in your engine for a moment.

Part of the car servicing process is to put xxx quarts of oil in the engine.

Think about possible failure modes associated with this process step.
  1. No oil
  2. Too much oil
  3. Insufficient oil
  4. Incorrect oil

Let's now take "No oil" as an example.
What would be the possible effects of this failure mode?
"Major engine damage."

If you did not have any oil lights on your dashboard display, you would have no way of knowing and the engine damage could possibly be beyond repair. (Without Warning) Severity 10.

If you have oil lights on your dashboard display, you would be able to immediately shut off the engine and limit the engine damage. (With Warning) Severity 9.

Simple non-high tech example. :agree1:

Hope it helps.

Stijloor.
