Form Controlled Document Usage

Good morning all. I have seen this through 3 decades in manufacturing, and consensus/ consistency is difficult to find.
Issue: Form Controlled Document (i.e. for used for Audit Schedule) being saved as the same control# after filling out.
This seems like an obvious error in my opinion, but I have seen it time and again.
The instance I am looking at currently is the above example; A Form Controlled Document, built as a yearly audit schedule, being saved as the FCD as a "completed" filled out form.
I have always held the thought that the blank form should remain as the only "Form Controlled Document", and that the completed (filled-in) form should be changed to something like an SOP or WI, after the form is filled out?
Some instances are more egregious than my example above, but even this doesn't sit well with my views of document control.
Thoughts?

I agree with Roxane. The form is controlled to ensure the record will be complete. One filled in, the form is now a record.

To the OP, consider how the records are retained (controlled). Look at your process against the requirements, and fill in the gaps.
 
And the record is controlled by the date it is completed. And any dates for individual recordings such as inspections performed on different lots and days.

Not only is this standard practice it makes the most sense and is the most efficient.
