Use Templates
Create the form and enter the form fields. Save the document as a template (.dot). Now, whenever the template is opened, it prompts you to save it as a Word document. For added protection, go to Windows Explorer, right-click on the file and select the read-only file attribute. Don't forget to password protect the template for forms before setting the attribute to read-only. It may also help if this template resides in a folder with permission levels set to read and execute for the rest of the company and set to full control for you. This allows you to control any modifications to the template. This will also allow you to set up your users so they can access the template through Word. Once a Workgroup Template directory has been established, place the templates in this folder. Then, have the users (in Word) go to Tools/Options/File Locations/Workgroup Templates. Then click modify. Specify the path for the template.
Now, all the users have to do in order to use the template is go to File/New in Word and select your template from the General tab. I use this method because it allows me to update the template without having to resend it out to everyone after the revisions are approved.
Stay away from Adobe. We have Adobe here but do not use it for forms. In order for the form feature to work (for everyone to have the ability to fill out an Adobe form online) each person must have the full Acrobat Writer application installed on their machine.
Word will work well for forms.
