Help in document control procedure

Hi all
This is my first post in this forum and I need help to develop a process for controlling documents within the organization due to numerous ‘out of date’ documents being identified in recent internal audits.
So I can develop a process flow chart detailing how to update and amend a manual for departmental representatives to follow.

That's not an ISO requirement so where does it originate and why? I've reviewed documents many years old that aren't out of date simply because of age.

Hey , just wanted to know more about this ... actually i partially agree to the point that documents need not be necessarily out of date due to its age in its lifecycle , during many surveillance audits , auditors have asked whey no revisions are there for certain procedures , that where maybe prepared 10 years back, most procedures remains the same as they really don't need any changes . but they still ask aren't annual reviews carried out etc, so is there any real requirement as per iso for this , as in to have regular reviews and revisions ? .
