How do you determine if hipot is necessary for a cable product?

I think it depends what type of device it is being used for. The device will have certain requirements.
Yes, true. I take that as a given.
But are there actual triggers that mandates a hipot test? Example: Voltage/current nears the cable ratings? Used near water (electrical safety)? Certain connectors used?

I think there is always a level of Hipot required for any cable carrying mains power. I think it's a design requirement.

All insulators will have dielectric strength requirements.

I seem to remember that low voltage cables had different requirements depending on if the user is in contact with it.

Medical had levels specified not only for voltage but also where it is on or in the body, if I remember from 30ish years ago.

A customer is asking the following. "How do you know when hipot testing is necessary for a cable product?"
Are you the design authority?

If not, when the customer sends the order, do you know intended use?

If not, seems to me the customer should be advising when to do the hi pot, or you could put it into your procedure to do it anyway.

Those CIRRUS testers are marvelous for this application, FWIW.

As far as I found it is crucial because it is the best way to uncover workmanship and assembly defects in an electrical product that can lead to insulation breakdown.

I agree but where does it say when hipot is required? Standard? A place that triggers an engineer saying it is required?
(Disclaimer - this my non-engineer understanding) "Required" is going to be driven by regulatory requirements for a product in a country or region. That regulatory requirement will likely specify a standard (e.g., 60601) that must be met in order to meet the regulatory requirement. The standard will specify the hipot testing requirements.
