I agree with simple structure in resolving a problem, however details often come into play where more granularity are necessary. I would advise books on 6sigma and especially DMAIIC methodology. J&J had a wonderful program of training on this with sponsors, problem, and monitoring.
DEFINE the problem is first and may take a week or more during which involvement of all knowing parties must be present. Once the definition is generally agree upon, gather related data, analyze it (with discussion), create solutions, test, and review data after changes have been made.
