How to define QCC (Quality Control Circle) Subjects

Due to an unstabile quality in our products, we want to have some QCC activity, but ,we have no idea to organizing a QCC activity.
Please inform us: how to define a subject ? which one should be involved? How to measure the effect?

I've made a corrective action by QCC activity , now ,i think 5-why method is very useful in such improvement .
