How to do a Root Cause Failure Analysis

Hi my fellow quality ferries

We just went through an audit and since we had to reschedule, we got different auditor. was not pretty. the guy did not understand the scope of our business.
so anyways, we got 4 minor ones- little babies easy to fix, but having trouble writing a root cause

any one is really good at those in here,
I need a help.

maybe it may be nice to give more info about your NC, it will be easier to discuss and establish a root cause. will not ?

Hi Moncia,

Our registrar doesn't require root cause analysis for minor nonconformances, only for the major ones. Sometimes, especially when they are easy to fix, a simple correction plan is sufficient, along with follow-up for verification that it was effective.

that's exactly what I have
easy - delete part of a sentence problem
but the guy was major pain in something
and I simply dont know what to write for simple easy to fix problems

Yes, please--exactly as they appear on the report, including the references to the requirements.

Established nonconformity (Requirement, Nonconformity, Evidence):
Requirement: Records to provide evidence of conformity to requirements and of the effective operation of the quality management system shall be controlled. The organization shall establish a documented procedure to define the controls needed for the identification, storage, protection, retrieval, retention time and disposition of records. Records shall remain legible, readily identifiable and retrievable.

Nonconformity: The records control process is not fully effective. The records matrix which is a key tool in controlling records does not contain all the record types in the QMS
Evidence: Records matrix used in the Records Control procedure 4.2.4 Rev 2 dated 7/17/09 does not contain the Purchasing record types.

Make copy and paste of the text as is, so that we can read what they reported.
If you want to add some other details too, we can better understand the dynamic of the audit.
Was the audit vs 9001 or other standard? Was it surveillance audit?
What is your business?
