Wait for other comments here.
In my case, we put dates of actual training, or target date/plan of training. And we document actual training in our Training Sheet.

I coming in late to this discussion, but after 2 years of trying to teach supervisors to do a root-cause analysis, I made up this work sheet that makes them look at human and environmental factors, then perform a basic "5 Why" RCA. It has proven to be a relatively effective tool to date.

if that was a case of one time training - we do the same

but we have to have like monthly training in here for people who do not know how to operate computer

that was exactly the NC - they were trained on accessing our system, they were told their ID's, passwords, how to use it
but some of our employees - specially the older ones just dont know how to use computer
so we need to have like constant reminder training type for them to make them comfortable working with computer

so it will be happening every other month until the next audit

they will be trained , re- trained, re-re-trained re-re-re-re-trained .... and so on
until they get it

Personally, I have found Apollo RCA to be overkill in all but the most complex RCA situations. I have done maybe 4 Apollo RCAs in the last 2-3 years out of probably 100 or more RCAs written. IMO it has a place, but it is not my most frequently used RCA tool.

Is the training more and more advanced each time, or are you going over the same subjects over and over again? If the latter, I'd look carefully at the training methods, trainer, etc. and/or the competence of the trainees.

I'd just write a work instruction. Detailed instructions on how to comply with a procedure are often put on a WI so the employees have step-by-step instructions handy.

May I suggest that you have a work instruction on how to access the files in the computer that you keep in hard copy near the computer?

May I also suggest that "ongoing" as in "forever" isn't really a good answer. Perhaps when the training fully implemented and the hard copy work instruction for accessing the rest of the work instructions is in place would work toward having some closure.

Yes I totally agree, On-going seems to be vague. How would you prove that the on-going action is helping the employees. How do you measure them. I guess having a hardcopy on how to access the computer is the best solution. I guess it would be less than 10 actions anyway..not that difficult to follow.
