IAQG-OASIS - 9101:2016 Form 4: Cause Code Listing Now Included in Form

Dear OASIS User,

You should be aware of the need for the certified organization to allocate a Cause Code when completing Section 2 of the Nonconformity Report (9101:2016 Form 4), for those audits conducted against the 9100:2016 series.

You may also be aware that the defined Cause Codes are built into the functionality of the Nonconformity Report input screen, within the OASIS Next Generation database.

For clarity, the MS Word version of the 9101:2016 Form 4 (accessed via the IAQG Forms Management section of the website: http://www.sae.org/iaqg/forms/index.htm ), has been updated to include the Cause Code list as part of the Form instructions and labeled as Annex A.

Please share this information within your respective organizations accordingly.
