What do you mean there is no evidence of effective planning of post-failure activities?

There is a column for Risk Plan and Controls and a column for Contingency Plan. On the tab labeled "risk matrix" at the bottom is a key detailing the types of risk controls to explain what types are applied for each failure.

Please elaborate on your concerns so I can review my document. Thank you!

Dear Lisa, have you seen my previous post here?
I put there 4 aspects which should be respected during creation of contingency plan. I treat very, very seriously word "PLAN", so I expect to see activity planning here.
What is triggering implementation of contingency plan?
What is next action following contingency plan launch?

Elaborating would take hours and require feedback from your organization, but let's start a small sample here.

Loss of Electric Power
If failure of external sources occurs, an on-site generator of approximately 2,0 MW would be rented and installed to continue manufacturing operations.

1. How do you define failure?
It is interruption with duration of how much or power drop value? Who's starts to count time from failure start? Is this person always on-site or notification is transferred to person outside his(her) working hours and (s)he counts?
2. Who is a supplier of generator? RMU? Who is contact person at supplier? Are there any working hours limitations? Who is in charge of delivering generator on-site? Is there a risk that generator won't be available, because of e.g. maintenance, other customer already ordering it?
3. How generator is connected to current site power system? Who is doing it?
4. How much does it take to restore power supply and does this information is used as input to production scheduling to ensure safety stock?

A lot more questions to ask and answers to get.

Requirements of IATF 16949 comparing to ISO/TS 16949 have escalated in this area, so it is a clear evidence for me, that there was something wrong with meeting this requirement in the past. Organization had established useless documents, relied too much on personnel (especially in maintenance area) competences and failed at the end.

We tried to keep it pretty generic in those areas due to the size limitations. We have only 19 employees here in this facility and the town we live in is less than 15,000. As an example, RMU is the town utility company, which provides us with the water, electric, trash, etc. We have limited departments and I typically refer to the Organization chart for responsibilities (see the column labeled "Responsible Group"). Since our organization is small, the group responsible is limited to a few individuals of which any absence would mean the entire group is trained and prepared to take actions.

Hello Lisa, like most who have looked at your contingency plan doc it's pretty robust and a great starting point. Everyone seems to want to use it as a template. Would you consider giving up the password to unlock it or posting an unlocked version of it for Forum use?

I have attached my most recent copy. This version was audited at our IATF upgrade audit and there were no issues found. The password for modification is "ag" (lowercase letters). Happy modifying!!!

Did Your auditor comment on testing of contingency plan?
My auditor just stated that all points must be audited annually.
If there is any information/comments to share I would appreciate it.
